Insurance
Please provide your insurance information to Vitalize at least 48 hours before your first visit. We will check your benefits to determine your coverage and what sort of copay or deductible you would be responsible for. Most insurance plans have a variety of tiers of coverage. Sometimes we will have to estimate your payment until we receive an Explanation of Benefits (EOB) from your insurance. Ultimately, it is your responsibility to know your insurance benefits and be responsible for payment if a service is not covered.

Sorry, we cannot currently accept any Medicaid or Medicare Plans. You may be able to submit your receipts (known as a superbill) for reimbursement.
